Clinic Policy
• All clients must undergo an initial medical consultation before treatment
• Full medical history, medications, allergies, pregnancy/breastfeeding status required
• Informed consent must be signed before each procedure
• Clinic reserves the right to refuse treatment if medically contraindicated
• Written consent required for all injectable, energy-based, and invasive treatments
• Risks, benefits, alternatives, and post-care explained clearly
• Consent renewed if:
• Treatment plan changes
• Significant time has passed
• New medical conditions arise
• Only licensed and trained professionals may perform treatments
• Procedures performed strictly within regulatory scope of practice
• Appointments confirmed in advance
• 24–48 hours notice required for cancellations
• Late cancellations or no-shows may incur a fee
• Chronic no-shows may require prepayment
• Payment due at time of service
• Packages are non-refundable but may be adjustable/ modified (clinic discretion)
• No refunds for completed services
• Refunds for prepaid services only if medically contraindicated
• Results vary per individual
• No guaranteed outcomes
• Multiple sessions may be required
• Touch-ups are not automatic unless specified in writing
• Clinic follows evidence-based emergency protocols
• Clients must report side effects immediately
• Follow-up care provided as clinically indicated
• Referral to medical services when necessary
• Clients must follow all pre-care and aftercare instructions
• Non-compliance may increase risk and void complimentary corrections
• Clinic not responsible for outcomes affected by non-adherence
• Before-and-after photos taken for medical documentation
• Photos stored securely and confidentially
• Separate consent required for marketing or educational use
• Clients may decline marketing use without affecting care
• All personal and medical information protected under privacy laws
• Records stored securely (electronic or physical)
• Information released only with written consent or legal obligation
• Single-use, sterile supplies where applicable
• Proper sanitation and disinfection protocols followed
• PPE used according to procedure risk
• Compliance with public health regulations
• Minimum age for treatments enforced
• Parental/guardian consent required for minors where permitted
• Some treatments restricted to adults only
• Respectful behavior required toward staff and other clients
• Harassment, aggression, or intoxication will result in refusal of service
• Clinic reserves the right to terminate client relationship if necessary
• Prescription products dispensed only following medical assessment
• Products are non-returnable once opened or dispensed
• Off-label use explained and documented when applicable
• Concerns should be reported promptly
• Clinic will investigate complaints professionally and confidentially
• Written complaints preferred for documentation and resolution
